Health services are delivered by a lot of different organisations. Some are public organisations like hospitals or private businesses like your local GP clinic or chemist. Others are not-for-profit organisations like an Aboriginal Medical Service. They all provide care to your community.
This survey will ask about your experiences with health services over the past 12 months. Your answers are anonymous and confidential. The survey should take around 10 minutes to complete.
All residents of Liverpool Plains over the age of 15 can take the survey. You can answer questions for yourself and others in your immediate family. If you are a carer, you can also speak on behalf of the person you care for.
The survey is being supported by Liverpool Plains Shire Council, Rural Doctors Network, local health providers, and healthcare support agencies. The aim is to improve access to healthcare in Liverpool Plains. A summary of the results will be published early next year.
